Is There Any Alternative Treatment to Steroids For FSGS

Q: I am a Doctor and I need some help to support one of my FSGS patient. He did take on very long term steroids, but then stopped them. May be he took these medicines three years in proper manner then he was only taking warffarin ,tab persintin and aspirin whole the time till recently he stopped them. Now he is taking tab loprin 150 mg BD,tab lophos 1 tab TDS after meals,tab mepressor 100 mg half tab BD ,tab qalsan (calcium) one tab OD, tab folic acid (iron) one tab OD, tab Alfa D one tab every two days 0.5 micro gram,injection S/C twice a week (10,000 units ), now will take steroids as per your recommendation waiting for your reply thanks for your kind reply.

A: You are also doctor, I think you should know FSGS is a kind of auto-immune disease, so it's easy for you to understand my words. Treating FSGS should aim to rebuild the disordered immune system.

Due to the disordered immune system, too much immune complex deposits on the glomerular capillary loop, resulting the segmental Hyaline degeneration of the glomerular epithelial cells, then under the further damage, the injured epithelial cells transform to myofibroblast, resulting into glomerular sclerosis, so the barrier function of the kidneys decreased, large amount of protein leak out into the urine.

How to get satisfied treatment result? Firstly rebuild the disordered immune system, then repair the injured epithelial cells to improve kidney function. In order to reach such aim, six treatment steps must be contained, that is Immune examination, Immune clearance, Immune blocking, Immune tolerance, Immune regulation and Immune protection.
