Polycystic Kidney Disease

PKD Basics

pkd basics

Cause

Mutations in the PKD1, PKD2, and PKHD1 genes cause autosomal dominant PKD and autosomal recessive PKD. Acquired cystic kidney disease (ACKD), another type of PKD develops on its own. Kidney failure causes ACKD.

Faulty genes cause Polycystic Kidney Disease and the genetic defective genes run in the families. There are two different genetic flaws that lead to two types of PKD. PKD1, PKD2 and PKHD1 genes cause ADPKD (autosomal dominant polycystic kidney disease) and ARPKD (autosomal recessive polycystic kidney disease).

Autosomal Dominant Polycystic Kidney Disease

ADPKD is the common type of PKD, which is affected by PKD 1 and PKD 2 genes. The faulty PKD 1 and PKD 2 gene lead polycystin-1and polycystin-2 proteins to generate. This causes the numerous cysts in the kidneys.

ADPKD is called adult PKD for it develops symptoms in one 30s and 40s. Children of ADPKD have at least 50% chance to get the faulty gene.

Autosomal Recessive Polycystic Kidney Disease

ARPKD is less common which makes up about 10% of all cases of PKD. ARPKD is caused by PKHD1 gene which leads to generation of fibrocystin or polyductin proteins. It is considered that the mutated PKHD1 causes the formation of kidney cysts.

ARPKD is known as infantile PKD for its signs occur shortly after birth. Both parents need to have the disease in order for it passes along to the children and each child has 25% of chance of inheriting the disease.

In some cases, a person is with PKD without familial history of the disease. But this condition is rare to see.

Prevention

● If you are considering having a baby, a genetic counselor can help you assess your risk of passing on the faulty PKD genes.

PKD Diagnosis

pkd Diagnosis

If you have Polycystic Kidney Disease, it is important to get an accurate diagnosis as soon as possible. This helps increase the odds for successful treatment and recovery.

Polycystic Kidney Disease Diagnosis

In order to make a polycystic kidney disease diagnosis, your doctor will ask a number of questions related to your medical history. This medical history will include questions about current symptoms, other medical conditions you may have, medications you are currently taking, and a family history of any conditions (including polycystic kidney disease).

Several diagnostic tests will help to check out the size and number of kidney cysts as well as to evaluate the amount of healthy tissues.

Ultrasound. Ultrasound scanning, a screening test, is a good tool to help doctor to diagnose polycystic kidneys. In this test, your doctor will put a transducer in your body which help reflect scanning your kidneys and show the pictures of your kidneys. If there is kidney mass in your body, it can contribute to making a straightforward diagnosis. However, it hardly makes an exact diagnosis in young children for renal cysts usually begin to grow up at one's 30s.

CT (Computerized Tomography) Scan. When you lie on a medical table, it will move and bring you into a big doughnut-shaped device which can project X-ray beams through your body. Through this process, your doctor can see cross-sectional images of your kidneys.

MRI (Magnetic Resonance Imaging) scan. It is a medical imaging technique which can make your internal structures, including kidneys, visible. It can predict functional changes earlier than standard blood and urine tests in people with PKD.

Gene test. There are at least six different genes which can cause different forms of PKD. As genetic test is available in many hospitals, it is popular than before. In this test, a blood sample should be taken. Some genetic test even can help parents to know whether their fetus is affected. The gene tests including: denaturing high performance liquid chromatography, RNAse, single-stranded conformational polymorphism, SSCP, systemic lupus Erythematosus etc.

Other recommendations include limiting fat intake to maintain a healthy weight.

Low-sodium

One of the major functions of our kidneys is to filter waste products from the body and regulate the fluid composition and blood pressure. When cysts burden your kidneys, you have a greater risk to develop high blood pressure. Low-sodium helps decrease the morbidity of hypertension. Table salt is a common origin of sodium. People with PKD are recommended to control the amount of sodium within 2.000mg per day.

Low-protein

Protein is the essential nutrition for human to grow and develop. However, as the final products of protein is needed to be filter out by kidney, people with PKD is suggested to control the intake of protein foods. Our physicians help make an assessment and determine whether you will benefit from a low-protein diet.

Fat intake can lead to arterial plaques, increasing the risk of high blood pressure and worsening kidney functions. Foods with 2 percent of fat or less are considered as low-fat foods. To maintain low-fat diet, milk and milk products, as well as processed foods should be avoided. Our dietitian can help provide a list of low-fat food for you.

According to a study published, caffeine should be avoided by those with PKD for it may interfere with the body's ability to control cyst formation.
