CKD Basics

CKD has been a worldwide worrisome health problem in recent years. CKD is defined as either kidney damages or decreased GFR of less than 60 mL/min/1.73 m2 over 3 month (The Kidney Disease Outcomes Quality Initiative (K/DOQI) of the National Kidney Foundation).

CKD can be categorized to 5 stages, ranging from nearly normal kidney function to renal failure, which denotes the total or near-total loss of your kidney function and requires dialysis or kidney transplant.

CKD Stage

According to the definition from the Renal Association, CKD can be divided into 5 stages:

Stage 1: normal or increased GFR﹥90 mL/min/1.73 m2 with protein in the urine

Stage 2: mild reduced kidney function (GFR: 60-83 mL/min/1.73 m2)

Stage 3: moderate reduction of kidney function (GFR: 30-59 mL/min/1.73 m2)

Stage 4: severely reduced kidney function (GFR: 15-29 mL/min/1.73 m2)

Stage 5: end-stage renal failure (GFR < 15 mL/min/1.73 m2 or on dialysis)

* All GFR values are normalized to an average surface area (size) of 1.73m2

CKD Symptoms

CKD has no signs or symptoms in the early stage; it only can be detected through lab tests.

Laboratory tests

Urine test: measure waste removal

Urinalysis: measure the kidney function by urinalysis. The urine is examined the red and white blood cells as well as solids.

24-hour urine tests: measure the severity of kidney damage and level of kidney function.

GFR: GFR is short for glomerular filtration rate. It is one way to measure the overall kidney function.

Blood test

Serum creatinine and BUN: serum creatinine measures the creatinine level in blood. Along with BUN, abnormal level of the tests indicates kidney disease.

If you are suffering from CKD, you had better follow a strict diet to help maintain a balance of electrolytes, minerals or fluids, and to stay away from waste build-up in your body.

Limit your protein intake

Lowering the protein intake will help your damaged kidneys to filter less waste so as to prevent more damages from occurring. The recommended dietary allowance to healthy person is 0.8 grams per kilogram. It is limited to 0.6 for people with CKD. Main sources of protein include meat, fish, eggs, beef and vegetables etc. Talk with your dietitian about how much protein you should consume every day, it is very important to monitor protein status by them.

The amount of calories intake varies from people to people. According to NKF, the amount should be 35 per kilogram per day. Get enough calories if you are losing weight, especially to people with diabetes. It is often found in carbohydrates, protein, fats and so on.

Restrict foods high in potassium, sodium, phosphorus etc. Foods high in potassium are bananas, oranges, milk, prunes, nuts and beans. Reducing sodium intake will help you control your high blood pressure. Less than 100 mg per serving is excellent. High phosphorus foods include milk, beans, lima beans, chocolate, yogurt, cheese, liver and others.

Fluid intake

You can drink as normal in the early stages of CKD, while fluid intake should be restricted as swelling occurs. The swelling in your face, ankles or feet can be a sign of fluid retention in your body. Carefully monitor your condition and keep close touch with your dietitian so that everything will be okay.

Take calcium supplement and vitamin to prevent bone disease or balance the electrolytes in your body. As for the details, ask your dietitian or doctor. People with advanced CKD may suffer from anemia and need extra irons.

The diet is not a cure for CKD, it is more likely a way to slow down the progression. The diet alone is unable to reverse the kidney function or the damage, but it can help delay renal failure from happening. The diet should change along with the condition of your kidneys. Whatever changes you want to make, talk it over with your doctor or dietitian.
