He knows all of these, so he accept the reality that he needs long term of regular dialysis.
At local hospital, he takes dialysis three times one week. Last year, he came to our hospital, Shijiazhuang Kidney Disease Hospital, and then he can take dialysis once a week.
When discharged from our hospital, the doctor told him to keep on dialysis once a week and take traditional Chinese herbs. However, after one month, Mr. Zhang came back to our hospital and insisted to take dialysis here. We asked him why he didn’t want to take dialysis in the local hospital. He said he was so confused because when he took dialysis at local hospital, he experienced low blood pressure and always feel fatigue that he never have in our hospital.
Is that because the dialysis machine is not advanced?
Our director of dialysis department, Tang Junting explained to him. Dialysis should vary from person to person. The differences include dialyzate formula, temperature, ultrafiltration profile, sodium profile, blood volume, and the use of anticoagulant,etc.
Dr. Tang says nowadays many nurses and doctor use dialysis machine simply. They just know how to turn it on and turn it off, rather than adjust it based on patient’s illness condition.
In fact, many problems can be solved by adjustment of dialysis. For example:
-Patients with low calcium level and muscle cramps can be relieved by high sodium dialyzate.
-Low blood pressure: Make less after more than a first rate curve. Then lower dialyzate temperature.
-Temporary low blood pressure on eating: Make a curve with more than two head, less in the middle. Reduce ultrafiltration rate while on eating.
-Difficult controlled high blood pressure: Adjust the requirement of dry weigh and change sodium concentration.
