Temporary Fever During DialysisHemodialysis fever is a complication of hemodialysis with fever as the main manifestation. It may appear within 1 or 2 hours after the start of dialysis or after the end of dialysis. Once fever occurs, the relationship with hemodialysis should be analyzed firstly. If it is indeed caused by hemodialysis, you should find out the cause and then take corresponding measures to prevent it.

Causes of fever during dialysis

1. In most cases, it is caused by pyrogen entering the blood. For example, the reuse of dialytic pipes and dialyzers is not standard, and the dialysate is polluted.

2. Sterile operation is not strict when dialysis so as to cause pathogen to enter blood, or original infection gets spread because of dialysis, causing fever.

3. Other rare causes, such as acute hemolysis, high temperature dialysis, can also cause fever.

Diagnosis of fever during dialysis

Fever is a kind of symptom that may appear in dialysis process. The diagnosis is to make clear the causes, which are mainly divided into infective fever and non-infective fever.

-For fever caused by pyrogen, temperature is usually normal before dialysis. 1 ~ 2 hours after dialysis, fever and chills present, and body temperature is between 38°~ 39°, which lasts 2 ~ 4 hours continuously. Blood culture is negative, blood leukocyte and neutrophil count do not to rise.

-For infective fever, temperature rises 2-3 days after dialysis to over 39. The white blood cells and neutrophil count significantly increase and it is positive in blood culture.

Treatment for fever during dialysis

1. For patients with high fever, first of all, symptomatic treatment should be done, including physical cooling, oral antipyretic drugs, and appropriate reduction of the temperature of dialysis fluid.

2. Blood culture should be done if it is bacterial infection, and then give antibiotic treatment. Generally, people who are caused by pyrogen get better within 24 hours. If they do not, they should be considered to be caused by infection. They should continue to look for pathogens and take antibiotics.

3. Non-infection-induced fever can be treated with low doses of glucocorticoid.

Prevention for fever during dialysis

1. The operation should be strictly regulated in dialysis operation, reuse of dialysis piping and dialyzer so as to avoid pyrogen pollution.

2. Disposable dialyzer and dialysis tube can be used if conditions allow.

3. To fully flush the dialysis line and dialyzer before dialysis.

4. To strengthen the monitoring of hemodialysis water and dialysate, and avoid using contaminated dialysate for dialysis.
