Uremia is not fatal, but mortality of cardiovascular disease is high. More than 50% of patients with chronic kidney disease (CKD) die of cardiovascular disease before they develop uremia.

Risk factors for cardiovascular disease in kidney patients include hyperphosphatemia, anemia, immune inflammation and oxidative stress.

Hyperphosphatemia is the excess of blood phosphorus, which can cause osteoporosis and systemic pruritus. Anaemia is due to low hemoglobin, which can make the person looks pale and dark yellow. High phosphorus and anemia are easy to understand, while immune inflammation and oxidative stress are more specialized and less easy to understand, but they do pose a great risk for kidney disease, increase the risk of cardiovascular disease, and threaten the safety of patients with kidney disease.

Recently, ascorbic acid (or vitamin C) has been found to reduce oxidative stress levels, which may improve eventual outcomes in patients with kidney disease: reducing cardiovascular disease risk and mortality.

Impaired endothelial function in patients with kidney disease can lead to an increase in asymmetric dimethylarginine, resulting in an increased risk and mortality of cardiovascular disease. Therefore, reducing the level of asymmetric dimethylarginine can improve the prognosis of kidney disease, which is the role of ascorbic acid.

In order to clarify the effect of ascorbic acid on chronic kidney disease and hypertensive oxidative stress and endothelial function, Keith Gillis from the university of Glasgow, UK, conducted a cross study of intravenous infusion of ascorbic acid and saline in 30 patients with renal disease. The control cohort was 20 patients with hypertension.

The results showed that the ascorbic acid level was significantly reduced in 22 patients with chronic kidney disease. Blood pressure decreased in both the nephrotic and hypertensive groups.

The results are published in Clinical Kidney Journal.

Is vitamin C a new drug for kidney disease?

Vitamin C can reduce the level of asymmetric dimethylarginine and oxidative stress, which is of great significance to patients with kidney disease. But at the moment, the trial is limited in size, and it has not been fully determined which kidney patients benefit from vitamin C use, which will require large randomized, double-blind, placebo-controlled studies to further determine.
