-Steroid

Glucocorticoid is the first choice for urine protein. But because it has a lot of side effects, it is only used when you have massive proteinuria.

What is massive proteinuria? It is different for different types of kidney disease patients. Sertoli cell nephropathy usually has a lot of proteinuria, while mesangial cell nephropathy such as IgA Nephropathy and Purpura does not have so much urine protein. For Membranous Nephropathy patients, when 24h urine protein is more than 3.5g or even 8g, steroid is needed, while for IgA Nephropathy patients, when 24h urine protein is more than 1g, steroid is used.

-Anti-hypertensive medications

And then warfarin can be used for long-term.

-Immunosuppressants

When the effect of steroids is not obvious, immunosuppressants will be used. For Membranous Nephropathy, which is steroid-resistant, immunosuppressants are usually used directly.

But note that:

1. Cyclosporin A should be no more than 5mg/kg.d. And it should be avoided by patients with interstitial fibrosis. Besides, do not use it with other renal toxicity medications.

2. Tacrolimus, cellcept and sulfur file purine must be avoided by pregnant women and lactating women.

3. When leflunomide is taken, contraception is recommended. When it is stopped for half year, fertility is considered.

4. If liver function is not good, do not use sulfur file purine and cyclosporin A.
