Overview of CGN (Chronic Glomerulonephritis)

Chronic Glomerulonephritis (CGN) refers to a condition in which glomerular and tubulointerstitial fibrosis occurs. This chronic kidney disease can lead to reduced GFR (Glomerular filtration rate) and retention of retention of uremic toxins.
How serious is Chronic Glomerulonephritis?
Chronic Glomerulonephritis is irreversible when it reaches to advanced sages (Stage 3, 4, and 5). In the early stage, there is no obvious symptoms, so people find they have Chronic Glomerulonephritis with later stages.
If the progression of this disease is not halted with treatments, the final result is End Stage Renal Disease (ESRD), which requires dialysis or kidney transplant.
How to diagnose Chronic Glomerulonephritis?
The National Kidney Foundation (NKF) defines CKD on the basis of either of the following:
● Evidence of kidney damage based on abnormal urinalysis results (eg, proteinuria or hematuria) or structural abnormalities observed on ultrasound images
● A GFR of less than 60 mL/min for 3 or more months
Five stages of Chronic Glomerulonephritis
● Stage 1: Slight kidney damage with a normal GFR (≥ 90 mL/min)
● Stage 2: Mild kidney damage with a mild decrease in the GFR (60-90 mL/min)
● Stage 3: Moderately decreased GFR (to 30-59 mL/min)
● Stage 4: Severe decrease in the GFR (to 15-29 mL/min)
● Stage 5: End Stage Renal Disease (ESRD) or Kidney Failure with GFR<15mL/min
Treatment for Chronic Glomerulonephritis
Treatment depends on the cause of the CGN. Most of the times steroids are the mainstay of the therapy. In Wegner's and SLE, azathioprine & cyclophosphamide may be used in severe cases. All these are empirical treatments and not curative treatment.
